Attestation Forms to Confirm Vaccine Eligibility Not Required

The Ministry of Health has communicated to all 34 public health units that attestation forms and doctors’ notes are not required to confirm vaccine eligibility. This is in response to the OMA advocating to the government that attestation forms and doctors’ notes to confirm vaccine eligibility (for example, to confirm an eligible health condition) are a barrier to vaccination and an inappropriate use of health-care resources.
