The ODB limited use codes are for the two indications of rheumatoid arthritis (LU # 499) and ankylosing spondylitis (LU # 498).

Please note the following:

• New requests for Enbrel for those two indications will no longer be accepted
• Renewal requests for Enbrel for RA and AS will be accepted and assessed as per usual
• New requests for Enbrel for other indications such as JIA and psoriatic arthritis will be accepted as per usual
