EAP & Limited Use

The ORA publishes forms and codes of significance to Ontario rheumatologists within this section of its website, as a service to members and the community.

Exceptional Access Program (EAP)

EAP forms are to be completed when applying for MOH funding for drugs for rheumatological diseases through Ontario’s Exceptional Access Program (EAP).

Limited Use Codes

Current LU codes are available online in a database maintained by the Ontario Ministry of Health.
