We have modified the 4 JIA forms and have posted them on the ORA website. The modified forms have the OHIP+ dedicated fax number on them, and they have an additional “box” to check “TFA mechanism previously used” next to “Renewal”.  These forms should be used for renewals sent in during 2018.
